The Lost Cases of 221B Baker Street is the sequel to its 2008 predecessor, The Lost Cases of Sherlock Holmes. It may also be referred to as "The Lost Cases of Sherlock Holmes Volume 2k," which sold over 135 units online and at top retailers. It introduces sixteen additional mysteries to be solved in England, where players will visit over forty locations, including the famous Stonehenge as well as Marlsbury Castle and more. In your travels, you must investigate and zero in on suspects until the guilty party is found.

You will search for hidden objects, strategically placed in busy scenes. The objects to be found are listed on the left side of the screen. If you've scoured the picture and are finding it difficult to find all of the objects, you can zoom in on the busy scene by clicking the mouse to turn it into a magnifying glass. If you find yourself really perplexed and cannot move forward, you can use the "hint" button to reveal the location of a challenging object. With each new level, hints may be added as you locate the smoking pipe.

As you collect your objects, some of them may be evidence and lead to a mini-game. There are over one hundred of these mini-games to solve, both within and in addition to your original quest. Some of them are to be solved within a hidden object scene, and others, such as the tile sliding puzzles, jigsaws and others are solved in between scenes.

Once a crime has been solved, you will get to watch an animation which shows what happened as well as how it was all figured out. Speaking of animation, players will find the graphics more advanced and vivid than the original Lost Cases of Sherlock Holmes, and it has retained its good quality as far as voice acting.
